Guest Passes

*Our guest passes for dances have gone online! Please use this digital form to collect your signatures and have it turned in by the MONDAY before the dance. You are welcome to instead use a physical copy, and please submit it to the Activities Center by the same deadline.*
 
To bring a guest to our dance, please follow these instructions:
1. Open the digital form linked on this page and fill out yours and your guest's information. Attach a photo of their ID, either a student ID or Government Issued ID. Send the form to YOUR parent or guardian's email.
2. Your parent or guardian will get an email from InformedK12. They will input their information and sign the form, and then send it to YOUR GUEST'S parent or guardian's email.
3. Your guest's parent or guardian will get an email from InformedK12. They will input their information and sign the form, and then send it to YOUR GUEST'S school administrator's email. If your guest is not currently a student, they will be able to skip the next step.
4. Your guest's school administrator will input their information and sign the form. It will automatically send to the PHS Activities Director.
5. Mrs. Gilmore will review your guest pass and send it to your PHS Assistant Principal. Once they sign, you will receive an email letting you know they have approved your guest pass. Only after you get this email can you go purchase yours and your guest's dance ticket.
 
Please review the rules below regarding guests:
  • All attendees are subject to search upon entry.
  • Students may not enter more than 30 minutes after the dance begins nor leave more than 30 minutes before the dance ends. Students may not leave and return for any reason.
  • The student and guest must attend the dance with each other and enter the facilities at the same time. If the guest arrives later than the student and the student has already entered, the guest may not enter. Guests will not  be allowed at the dance without their host.
  • Tickets or guest passes are not interchangeable or refundable. Pass must correspond with the student's ID or they will not be admitted. All students and their guests must show their IDs to enter the dance.
  • Removal from the dance may be a result from one or more of the following: possession of or under the influence of drugs/alcohol, fighting, failure to follow direction of chaperones, or any other form of disrespect or inappropriate behavior.
  • Administrators can randomly administer a breathalyzer to students. Law enforcement may be contacted.
  • PHS students are responsible for their guest’s behavior.
  • Dress code is still in effect.
  • Students are expected to dance appropriately.
  • Students must be picked up or leave immediately following the dance.
  • No guests allowed over the age of 20. There are no exceptions.
