How to Participate

What is my Username?

• Your username is the email address that you used to register at the Home Campus login page.

What if I play multiple sports?

• Once you complete a clearance for one sport and reach the Confirmation Message, you will have the option to check off additional sports or activities for the current school year.
• If you decide to participate in an additional sport later on, you can access the multiple sport check boxes by clicking "Print" under the Confirmation Message of your original clearance for that specific year.

What do I do about physicals?

• The physical form can be downloaded above. This form must be filled out by your doctor.
• We will also have a doctor on campus to provide physicals for $25 (cash only). Please check this page for upcoming dates.
• We accept physicals online by uploading the completed form in Step #1 above. You must also turn in a hard copy to the athletic department.

Why haven’t I been cleared?

• The school will review your submitted information and either Clear or Deny the student for participation.
• You will receive an automated email notification once the student has been cleared.

What do I do if my sport is not listed?

• Please contact the school’s athletic department to request that your sport be activated.

We are excited to announce that the CUHSD Board of Trustees approved an agreement with the Kyle J Taylor Foundation to provide Heart Screenings for athletes. All 9th and 11th grade athletes are required to have a heart screening as part of their athletic clearance.

Why this change?

Sudden Cardiac Arrest (SCA) is the leading cause of death for student athletes in the U.S. and on school campuses. Electrocardiogram (ECG) heart screening is the most effective tool to identify youth at risk. Given that 75% of student athlete fatalities are cardiovascular-related, these screenings are a vital component of the clearance process.

What do 9th and 11th grade athletes need to do?

Athletes in grades 9 and 11 must obtain a heart screening prior to participation in sports:

• If your outside doctor provides this screening during your physical, that meets the requirement.
• Alternatively, you can complete the screening for $10 at a campus event hosted by the Foundation.

What does this mean for the clearance process?

• Use the updated physical form found on this page; it includes the heart screening requirement.
• Information regarding on-campus heart screening events will be communicated through school channels.
